INSTRUCTIONS |
To complete PDF form using Acrobat Reader®: Open this file in Adobe Acrobat Reader®. Select the "hand" tool and move over the form until the hand changes into an I-bar or pointing finger. Type in your information, moving through the blue highlighted form fields using your TAB key or the cursor. Print the form and be sure to print a copy for yourself. If you do not have the Adobe Acrobat Reader, you may download it free from Adobe's Web site.

To complete Microsoft Word® form: Download this file and open it in Microsoft Word® (the form was created with Word 2003). Under the "File" menu choose "Save As," rename the document and save it to your computer. Place your cursor in an entry field and begin typing, using your TAB key or cursor to move between fields to fill in and complete the form. Save your document and print.
