Instructions to complete in Acrobat Reader®: Open this file in Adobe Acrobat Reader®. Select the "hand" tool and move over the form until the hand changes into an I-bar or pointing finger. Type in your information, moving through the blue highlighted form fields using your TAB key or the cursor. Print the form and be sure print a copy for yourself.
