Instructions to complete in Acrobat Reader®: Open this file in Adobe Acrobat Reader®. Select the "hand" tool and move over the form until the hand changes into an I-bar or pointing finger. Type in your information, moving through the form using your TAB key or the cursor. Print the form (making a copy for yourself), attach a current curriculum vitae, and submit the application to your dean. If you do not have the reader, you may download it free from Adobe's Web site.
